Healthy Farmer Program offers three key assessment packages Silver, Gold and Platinum. Each of these packages have been designed to provide any adult between 18 and 65 years of age with a health profile that includes a list of medical and lifestyle risks. Silver Package
This is a 12 month health program that commences with a self directed online health assessment and includes web base goal setting and behavioural change guidance.

Family Pack (4) $150 + GST
Gold Package
The Gold Package adds another layer of support to the Silver Package by providing access to a phone base health consultant. This person will call you to discuss your health report and your goals and provide some coaching support.

Platinum Package
This is a clinical assessment by a GP and health professional at a designated Ford Health clinic. This is ideal for people 40 years and older – especially those farmers that never go to their local GP. It includes Healthy Change Program which is a 12 month support program.
