Healthy Farmer Program offers three key assessment packages Silver, Gold and Platinum. Each of these packages have been designed to provide any adult between 18 and 65 years of age with a health profile that includes a list of medical and lifestyle risks. Silver Package

This is a 12 month health program that commences with a self-directed online health assessment. It includes access to the 'myhealth' program with online goal-setting and support tools, health information and your health record.

Gold Package

The Gold Package adds another layer of support to the Silver Package by providing phone coaching. Your health coach is a degree trained health professional who will motivate and assist you in managing your health.

Platinum Package

This 2 hour clinical assessment conducted by a GP and health professional in a designated Ford Health clinic is ideal for those aged 40 years and older. The 12 month online ‘myhealth’ program and phone coaching is included in this package.
